Incident Report

If you hear/see something, please log the concern here. Inform the student/professor that you are elevating the concern to the appropriate person to investigate. When necessary, take notes, and document everything as accurately as possible. DO NOT ADD COMMENTS OR OPINIONS (e.g. “XX was aggressive” if the person did not say “aggressive.”)

If the student or professor requests a specific action be taken, let them know that their request will be conveyed to whomever is authorized to investigate and respond.

If the student or professor is speaking on behalf of someone else, let the individual know that someone will contact the individual for details.

While every effort may be made to maintain confidentiality, there is no assurance of anonymity. If the issue involves a personnel matter, it may not be possible to inform the person filing the concern will be informed of any action.

In case of public safety, law enforcement or other outside agencies may be notified.
